1. Position: Program Specialist
• Number of Vacancies: 1
• TEAM/PROGRAMME: HIV and AIDS
• LOCATION: National Center for AIDS and STD Control
• GRADE: 2
• CONTRACT LENGTH: until March 15, 2021
• CHILD SAFEGUARDING:

Level 3: the post holder will have contact with children and/or young people either frequently (e.g. once a week or more) or intensively (e.g. four days in one month or more or overnight) because they work country programs; or are visiting country programs; or because they are responsible for implementing the police checking/vetting process staff.

ROLE PURPOSE:
The Program Specialist will work under the supervision of Sr. Program Manager and work in close collaboration with focal persons of NCASC for implementing HIV prevention, treatment and care and would support for developing the competency of the NCASC program team with relevant approaches that relate to achieving the care and treatment goals stated in the National HIV Strategic Plan 2016-2021.

The Specialist would also support to build the capacity of health care workers by providing HIV related training. The Specialist will also have to do supportive monitoring and onsite coaching to Service Delivery Points like HIV testing centers, PMTCT sites, ART centers, and OST sites.

In the event of a major humanitarian emergency, the role holder will be expected to work outside the normal role profile and be able to vary working hours accordingly.

2. Position: Genexpert Service Engineer
• TEAM/PROGRAM: TB Program Management Unit, Global Fund
• LOCATION: National Tuberculosis Centre, Bhaktapur
• GRADE: 4
• POST TYPE: National/Contract

Child Safeguarding Level 3 – the responsibilities of the post may require the post holder to have regular contact with or access to children or young people

ROLE PURPOSE:
Tuberculosis is still one of the leading causes of morbidity and mortality in Nepal despite the fact that it can be cured with adequate treatment. The emergence of multidrug-resistant tuberculosis (MDR-TB) is one of the challenges in our efforts to control TB. It has been estimated that each year about 45,000 new TB cases occur annually in Nepal and this estimation is likely to increase considerably as indicated by preliminary results of the ongoing prevalence survey. Although medical laboratories are crucial in making an evidence-based diagnosis of TB, they have long been a less prioritized component of the health care system in Nepal.

Introduction of Cartridge Based Nucleic Acid Amplification Technology (i.e. GeneXpert technology) to TB control programs has added great value in terms of diagnosis and treatment because of its higher sensitivity to detect TB and ability to detect resistance to rifampicin simultaneously. About 80 GeneXpert machines are already in use under the National Tuberculosis Program (NTP) in different parts of the country. Furthermore, there is a plan for increasing accessibility to GeneXpert services gradually throughout the country and replacing traditional diagnostic tools with GeneXpert technology.

It is crucial to have a functioning laboratory quality assurance (QA) and accreditation system to ensure the quality of GeneXpert testing in healthcare facilities. It is essential to track the functionality of GeneXpert machines across the country and a robust mechanism for troubleshooting and regular maintenance is important. Regular analysis of utilization data is required to inform NTP for further expansion of the services across the country. A GeneXpert Service Engineer is required to fulfill all these requirements and providing technical support and monitoring to laboratories providing GeneXpert services.

3. Position: Provincial Laboratory Officer
• TEAM/PROGRAM: Global Fund
• LOCATION: Provincial Public Health Laboratory (PPHL) in Sudur Paschim Province and Province 2
• GRADE: 4
• POST TYPE: Provincial/Contract

National Strategic Plan for Tuberculosis Prevention, Care and Control 2016-2021 envisions strengthening existing TB laboratory network and increasing accessibility and utilization of laboratory services across the country. GeneXpert testing (Cartridge-based Nucleic Acid Test) has now been given priority as a diagnostic tool and its expansion and optimal utilization have become important. Functional laboratory quality assurance systems are important to improve TB case finding and to ensure appropriate treatment.

It is crucial to have a functioning laboratory quality assurance (QA) and accreditation system to ensure the quality of TB laboratory services in healthcare facilities. Nepal now has a federal structure and comprises seven provinces. Each province has a Provincial Health Directorate under the Ministry of Social Development. Each province will also have a Provincial Public Health Laboratory which will be responsible for ensuring the quality of laboratory services within a province. A TB Laboratory Officer is needed in Sudur Paschim Province and Province 2 to provide support to the Provincial Public Health Laboratory for strengthening TB laboratory networks within the province and ensuring the quality of TB laboratory services. The TB Laboratory Officer will provide support to provinces for revitalizing sputum microscopy QA/QC and establishing Quality Assurance and accreditation system GeneXpert services.
